Use the form to apply for or enquire about a tax file number for an individual. To ensure the integrity of the tax file number system, the form must be accompanied by original documents which prove your identity. As a result, you cannot apply for a tax file number online at this time. Your TFN will not be processed without the necessary Proof of Identity documents. A full and revised list of acceptable documents is displayed on the back page of the form. Also it is important that you read the Privacy Statement on the back page before you sign the application form.

What is Land Tax?
Land tax is a tax levied on the ownership of land in NSW as at mid-night on 31 December of each year. It includes vacant land or a house, flats, home units and holiday homes. In general, your principal place of residence or land used for primary production is exempt from land tax.

The information you provide on the declaration will help your payer work out how much tax to take out of payments to be made to you.
This declaration covers payments for:
- work and services - payments to employees, company directors, office holders, as well as payments under return-to-work schemes, labour hire arrangements, or payments specified by regulation
- benefit and compensation payments
- retirement payments and annuities and eligible termination payments.
The entity making the payment is your 'payer'. You are the 'payee'. You should complete a new Tax file number declaration before you start to receive payments from any new payer – for example, when you start a new job or become entitled to a superannuation pension. Your payer must notify the Tax Office within 14 days of the start of the new arrangement.
